Hypochondriasis
-fear of having a serious disease based on the misrepresentation of bodily symptoms
Prevalence of hypochondriasis
0.8-4.5%
Treatment of hypochondriasis
- CBT is first line
- SSRIs may help
Body Dysmorphic disorder
- convinced that part of their body has a defect or is flawed in some way. The observer cannot see much
- has both psychotic and non-psychotic variants (delusional or without delusional)- insight is the key
Factors predisposing to BDD
- low self esteem
- critical parents and partners
- early childhood trauma
- unconscious displacement of emotional conflict
BDD and other disorders
- earlier onset of depression
- lifetime rate of major depression is 26%, social phobia 16%, OCD 6% and psychosis is higher
Cosmetic surgery in BDD
-rates of 7%-15% of patients seeking cosmetic surgery and 12% seeking dermatology have BDD
Treatment of BDD
- high dose SSRIs used for a long time (fluoxetine)
- CBT may help
Prognosis of BDD
- generally poor with waxing and waning course
- preserved psychosocial functioning is usually preserved
Somatisation disorder prevalence
1-2% in primary care
-twice as high in women than in men
Medically unexplained symptoms
- 42% had somatoform disorder
- 36% had depressive disorder
Somatic Symptom Disorder
-maladaptive thoughts, feelings and behaviours associated with somatic symptoms
Treatment of somatic disorders
- CBT< antidepressants and collaborative care
- Body orientated psychological therapy, mentalisation based CBT and brief dynamic interpersonal therapy
Conversion disorder
-ICD classifies this as a dissociative disorder
Dissociative disorder
10% adult population may have this
- appears to be as common as anxiety, mood and substance missuse disorder
- no gender difference
