Trazodone (Desyrel)
A
- useful for refractory major depression, major depression with anxiety, and INSOMNIA (secondary to its sedative effects)
- inhibits presynaptic 5HT reuptake and blocks postsynaptic 5HT2, and has antihistaminergic properties, used as sleep aid
- risk: PRIAPISM, SEDATION, ORTHOSTATIC HYPOTENSION, nausea, dizziness, cardiac arrhythmias

Mirtazapine (Remeron)
A
- useful in the treatment of REFRACTORY MAJOR DEPRESSION, especially in patients who need to GAIN WEIGHT
- side effects= sedation, WEIGHT GAIN, dizziness, somnolence, tremor, dry mouth, constipation and rare agranulocytosis
- no sexual side effects and few drug interactions
- Alpha2 blockade and NaSSA (noradrenergic and specific serotonergic antidepressant)
- 5HT1A agonist: antidepressant/anxiolytic benefit
- 5HT2A: sleep
- 5HT2C: wt gain
- H1 antagonist: sedation, wt gain, anxiolytic
- LESS ANTIHISTAMINE AND MORE NE EFFECT AT HIGHER DOSES
- INDICATIONS: MDD, ANXIETY
