Risk factors for gastroschisis
young maternal age, smoking and use of vasoactive agents
What is gastroschisis?
Fetal gastroschisis is defined as full thickness anterior abdominal wall defect with
bowel protruding through the defect
It usually occurs on the right side of a normally inserting umbilical cord
The defect is not covered by membrane
Special considerations if gastroschisis identified
10% still birth rate
If no other abnormalities, likelihood of genetic cause or syndrome is low
Once born, cover with a plastic bag. May be suitable for primary closure. Need to deliver in a unit with paed surgery.
Chorioid plexus cyst
Incidence 1-3%
Soft marker- most fetuses will not have an abnormality but there is an association with trisomy 18
Should have tertiary anatomy scan
Echogenic bowel
- If found in 3rd trimester- no significance
- Second trimester- in a normal fetus has no sequelae
- Can however be associated with increased risk of chromosomal and non chromosomal fetal
abnormalities - Causes: - Genetic e.g. aneuploidy
- Non genetic: intra-amniotic bleeding, CF, congenital malformations of the bowel e.g. Hirschprungs, IUGR, congenital infection
Need increased surveilance due to increased risk of IUGR and fetal death
