Paracetamol overdose

• When to test
• What to give and when?

what can you give for benzodiazapine, ethylene glycol (antifreeze), asipirin, opiod over dose?

Answer 1

Less than 4hr

• Less than 4 hours - give activated charcoal
• Test paracetamol level at 4h –> normogram plot
• Give n-acetylcysteine if on or above treatment line

4-8hr
-Test levels –> normogram, treat if over line

8-24 hr

• start n-acetyl cysteine
• check paracetamol level
• continue NAC if above the treatment line, stop if below

When to stop NAC - if asymptomatic

• conc is non-detectable
• ast and ALT normal or decreased if was elevated

Benzo- Flumazenil
Antifreeze - ethanol/dialysis
Aspirin - sodium bicarbonate
Opiods - naloxone
