What is Barrett’s esophagus?
Barrett’s esophagus refers to a metaplastic epithelial process world the squamous cells change to intestinal epithelium with columnar cells and goblet cells. Dysplastic changes increase the risk of developing esophageal adenocarcinoma from 1.2/1000 patient years to 5.1. Caucasian males are most affected, more so if chronic Gerd. Asymptomatic presentations are common. Pathogenesis includes effects of gastric acid and bile inducing inhibition of epidermal growth factor receptor with generation of NFkB, and activation of the homeolbox gene CDX2.

What is the treatment of Barrett’s esophagus?
NSAID treatment may help prevent dysplastic change but PPI’s do not. Once recognized regular endoscopies, biopsies, staging, and gastroenterological consideration of treatment options- submucosal resection, laser therapy, radiation, chemotherapy, or photodynamic therapy.
