Osteoporosis Flashcards
What is bone remodeling
Requires a balance between bone formation by osteoblasts and bone resorption by osteoclasts
what is osteoporosis
condition characterized by loss of bone density & subsequent fractures
osteoporosis is sometimes termed…?
silent disease
what is most affected in osteoporosis
wrist
hip
vertebral column
In osteoporosis is the bone more dense or porous
Porous
who is more likely to have osteoporsis….women or me
Women
why are women more likey to have osteoporosis
lower calcium intake
less bone mass/smaller frame
bone resorption begins earlier and accelerates at menopause
Increased longevity
Risk Factors of osteoporosis
Family Hx
Low body weight <128lbs
corticosteroid use >3months
advanced age
low calcium diet
Lack of weight bearing activity
Smoking
excessive alcohol
Clinical manifestations of Osteoporosis
Compression fracture - break in vertebral body of bone due to loss of bone mass. May cause back pain
AND
Progressive Vertebral Deformities
number of vertebrae: Cervical Thoracic Lumbar Sacral Coccyx
7 12 5 5 4
T/F Osteoporosis is preventable
TRUE
Adequate calcium intake for osteoporosis
1000mg pre menopausal and post menopauseal taking estrogen
1500 mg for post menopause W/O taking estrogen
2 most common calcium supplements
carbonate and citrate
What is the preferred method of taking calcium supplements
chewable supplements; take with food
How does the body tolerate large doses of calcium
difficult to absorb in single doses > 500 mg
What should also be taken with calcium
Vitamin D (800-1000 UI daily)
What 3 things do we want to promote in the management of osteoporosis
Load- bearing exercise
-30 min, 3 x week
Avoid tobacco and excessive alcohol
Corset to prevent vertebral collapse
Class of Drugs for osteoporosis
Bisphosphonates
Selective estrogen receptor modulator (SERM)
MOA of biphosphonates
inhibit osteoclast resorption
Bisphosphonate drugs
alendronate (Fosamax)
ibandronate (Boniva)
Nursing considerations when taking biphosphonates
take on empty stomach with full glass of water
take 30 min before food/meds
Remain upright x 30 min to prevent esophagitis
MOA of SERMs
mimic estrogen effects on bone by reducing bone resorption w/o stimulating breast/uterus
SERM drug
raloxifene (Evista)
