Treatment of osteoporosis - Antiresorptive therapy

A
  1. Calcium (1200-1500mg in >65)
  2. Vit D supplementation
  3. Smoking cessation; Decrease ETOH; Fall prevention; regular weight bearing exercise; hip protector
  4. Bisphosphonates (risendronate and alendronate)
    - Denosumab (RANKL)
    - Strontium ranelate
  5. Oestrogen
  6. Selective oestrogen receptor modulators (Raloxifene)
  7. Calcitonin (inconsistent results)

Treatment of osteoporosis - bone formation therapy

A
  1. PTH peptide
  2. Men with OP & androgen deficiency should be given androgen replacement unless there is a hx of prostate cancer
  3. Steroid induced OP: Calcium & Vit D in combination
  4. Surgical treatment of fractures

Investigations in Osteoporosis

A
  1. DEXA
  2. Bloods: calcium; ALP; Vit D; PTH
  3. Serum electrophoresis, FBE & ESR to look for evidence of myeloproliferative disease
  4. Renal & LFTs to exclude hepatic & renal disease
  5. TSH to exclude thyrotoxicosis
  6. Serum testosterone in men
  7. Skeletal x-rays
  8. Iliac crest bone biopsy with double tetracycline labelling
  9. Tests for secondary causes of OP
