What is the pathophysiology of osteomyelitis & what are the most common causes?

A

Osteomyelitis is infection & inflammation of the bone/ bone marrow.

Due to metastatic haematogenous spread or local infection.

Most common cause: Staphylococcus aureus

  • Haemophilus influenzae
  • Salmonella in sickle-cell patients

What is the clinical presentation of osteomyelitis?

A
  • Fever
  • Local pain
  • Erythema
  • Sinus formation (in chronic osteomyelitis)

What investigations would you do for osteomyelitis?

A
  • CT/ MRI/ bone scan (lytic centre with ring of sclerosis)
  • Bone biopsy & culture

A

4-6wks (start IV);

  • Flucloxacillin
  • Fusidic acid
