What is the px of OSD?

A
  1. Occurs during the adolescent growth spurt before the tibial tuberosity has finished ossification.
  2. In active children - strength of the quadriceps exceed the ability of the tibial tuberosity to resist that force
  3. contractions of the quadriceps muscles > multiple small avulsion fractures
  4. avulsed fragments heal and grow
  5. tibial tubercle enlarge

A
  • M: 12 and 15 years
  • F: 8 and 12 years

What is the presentation of OSD?

A
  • gradual onset of pain and swelling below the knee
  • relieved by rest, worsen on knee extension against resistance
  • tenderness and swelling at the tibial tuberosity

What are the differential diagnosis fo knee pain?

A
  • bone tumour

Knee pain associated with systemic symptoms

  • Septic arthritis
  • Juvenile idiopathic arthritis

Knee pain associated with an abnormal examination of the hip

  • perthe’s disease
  • Slipped proximal femoral epiphysis

Knee pain associated with injury

  • meniscal injuries
  • collateral and cruciate ligament injuries
  • tibial tuberosity fracture

A

General

  • Rest & Ice
  • Physiotherapy
  • self-limiting by 2-3 weeks

Pharmacological

  • paracetamol or ibuprofen
