What is OSD?
A
- self-limiting disorder of the knee found during adolescence

What is the px of OSD?
A
- Occurs during the adolescent growth spurt before the tibial tuberosity has finished ossification.
- In active children - strength of the quadriceps exceed the ability of the tibial tuberosity to resist that force
- contractions of the quadriceps muscles > multiple small avulsion fractures
- avulsed fragments heal and grow
- tibial tubercle enlarge

Which age groups are affected by OSD?
A
- M: 12 and 15 years
- F: 8 and 12 years

What is the presentation of OSD?
A
- gradual onset of pain and swelling below the knee
- relieved by rest, worsen on knee extension against resistance
- tenderness and swelling at the tibial tuberosity

What are the differential diagnosis fo knee pain?
A
- bone tumour
Knee pain associated with systemic symptoms
- Septic arthritis
- Juvenile idiopathic arthritis
Knee pain associated with an abnormal examination of the hip
- perthe’s disease
- Slipped proximal femoral epiphysis
Knee pain associated with injury
- meniscal injuries
- collateral and cruciate ligament injuries
- tibial tuberosity fracture

How would you mx OSD?
A
General
- Rest & Ice
- Physiotherapy
- self-limiting by 2-3 weeks
Pharmacological
- paracetamol or ibuprofen
