- Bed height suited to me or bed height so patients feet are on the floor
. - Patient left sat on the side of the bed without an adequate base of support (feet off the floor)
. - Vulnerable patients left unattended whilst in a compromising posture on the bed (e.g. a stroke patient left unattended whilst sitting on the side of the bed).
. - Lack of Consent – consent should be sought at the start of the assessment
. - Placement of patient or therapist into a harmful position
. - Breaks left off bed, or wheel chair (during transfers)
. - Items left on the floor constituting a trip hazard
. - Patients left with socks on during transfers
. - Practice which increases the risk of infection transmission (E.g. pillows left on floor)
