How should you prepare your supplies after you open your sterile kit.
Before opening it, do hand hygiene then open it w/ the 1-inch border
1) Pour the cleaning solution (chlorhexidine) into the cup filled with cotton balls
2) Hand hygiene then put on sterile gloves
3) Open the catheter kit by the innermost layer
- Just rip it open
4) Get 10cc syringe ready & attach it to the balloon port
5) Open lube package
- Drop it in edge of container
Outline the steps prior to insertion.
1) Have pt positioned appropriately
- Put blue pad under pt
2) Have drainage system ready to go at the end of the bed
3) Ensure the kit is open & sterile gloves are on
4) Drape pt w/ white drape
5) Grab second blue drape and expose the area w/ the middle of it
6) Put lube at end of Foley catheter
- 1-2in for female
7) Put non dominant hand on female genitalia
- No longer sterile!!! -> so it needs to stay put
- If it’s hard to work w/ one hand, you may need 2 people
- Spread the labia w/ 2 fingers
8) W/ the cotton ball & cleaning solution, use the forceps to clean from top down
- Until pt is visibly clean (3-6 times)
9) Grab tip of catheter
- As you’re inserting it, look at the other end b/c urine will come out of it. Make sure that end is in the tray or it will spill
What are the steps to inserting an indwelling catheter in a female patient?
1) Find urethra
2) Insert 2-3 inches or until you see urine come back then add another 1-2 inches
3) Continue to leave L hand on pt
4) Grab other end w/ R hand & inflate balloon w/ the syringe
5) Once balloon is inflated, you can let go of L hand
6) Then separate syringe from Foley catheter
- There will still be urine flowing out of this so grab the connection of the collection piece & attach the end of the Foley into it
What is done after the insertion of an indwelling catheter?
1) Rip off drapes
2) Clean up pt
3) Secure catheter (leg securer)
4) Make sure the catheter is below the level of the pt (side of the bed)
5) Make sure pt is comfortable
6) Cover them up
7) Document
8) Record urine output at end of shift
How much should you advance the indwelling catheter in male patients?
Advance it 7 to 9 inches or until urine starts draining, then advance it another inch (2.5 cm). If you meet any resistance, rotate or withdraw the catheter slightly.
