Is patient talking?
Are there signs of airway obstruction? Partial. Total - silence - no respiratory effort.
Laboured or Paradoxical breathing?
Are there abnormal sounds? Gurgling, snoring, strider?
Feel over mouth with hand for breath.

Question 2

Breathing

Answer 2

Respiratory Rate, pattern, depth? - Symmetry.
Saturations?
Skin colour? Cyanosis - central and peripheral. Mental status?
Effort/workload - Accessory muscles. Nasal flare. Pursed lips.
Coughing and sputum.
Pain on breathing.

Question 3

Circulation.

Answer 3

Heart rate - plus regularity, strength, rhythm. Chest pain?
Blood pressure - plus pulse pressure.
Capillary refill time - plus peripheral temperature.
Central body temperature. Cold, hot, sweaty, clammy,
Patient colour - pale, flushed, normal.
Fluid assessment - skin turgor, mucous membranes dry/oedema?
Fluid balance - input/output. Fluid balance chart.

Question 4

Disability.

Answer 4

AVPU or GCS.
Blood glucose.
Pain assessment.
Review medications.

Question 5

Exposure.

Answer 5

Bleeding.
Fluid overload. 
Allergy.
VTE.
