A

CPITN (community periodontal index of treatment needs) - used for BPE, ball ended with 0.5mm diameter, one black band extends from 3.5mm to 5.5mm
PCP12 - used for a full periodontal pocket chart, has two black bands at 3mm intervals

7 o clock - all surfaces of lower anterior teeth
9 o clock - buccal surfaces of UL and LL teeth, lingual surfaces of LR teeth
11 o clock - the rest (all surfaces of upper anterior teeth and UR teeth, lingual surfaces of UL and LL, buccal surfaces of LR teeth)
