Orthopedic disorders in childhood Flashcards
1
Q
Variations of normal posture
A
- Bow legs (genu varum)
- Knock knees (genu valgum)
- Flat feet (pes planus)
- In-toeing and out-toeing
- Causes of in-toeing = metatarsus varus, medial tibial torsion, persistent anteversion of femoral neck
- Out-toeing uncommon; when bilateral, often due to lateral rotation of hips and resolves spontaneously - Toe-walking (usually from habit; exclude mild CP, tightness of Achilles tendon, or inflammatory arthritis in foot or ankle; exclude Duchenne muscular dystrophy in older boys)
2
Q
Abnormal posture - causes
A
- Talipes equinovarus (clubfoot - foot inverted and in plantarflexion)
- Vertical talus (rocker-bottom foot)
- Talipes calcaneovagus (foot everted and dorsiflexed)
- Flat feet (due to tendo-Achilles contracture, tarsal coalition or inflammatory arthropathy)
- Pes cavus (high arched foot, often associated with neuromuscular disorders)
___ - DDH
- Scoliosis (lateral curvature of front plane of spine - idiopathic, congenital, secondary)
- Torticollis (wry neck - due to sternomastoid tumour in infants or muscular spasm/ENT infection/spinal tumour/cervical spine arthritis or posterior fossa tumour)
3
Q
Chronic painful limb, knee and back - causes (3)
A
- Growing pains (nocturnal idiopathic pain, age 3-12y)
- Hypermobility (older children or adolescents)
- Complex regional pain syndrome
4
Q
Acute onset limb pain - causes (3)
A
- Osteomyelitis
Malignant disease
- Malignant bone tumours (osteogenic sarcoma)
- Osteoid osteoma (benign tumour)
5
Q
Painful knee - causes
A
- Osgood-Schlatter disease
- Chondromalacia patellae
- Osteochondritis dissecans (segmental avascular necrosis of the subchondral bone)
- Subluxation and dislocation of the patella
- Injuries
6
Q
Back pain - causes (5+3)
A
- Mechanical causes - muscle spasm or soft tissue pain from injury
- Tumours - benign or malignant
- Vertebral osteomyelitis or discitis
- Spinal cord or nerve root entrapment
- Scheuermann disease
- Spondylolysis
- Spondylolisthesis
- Complex regional pain syndrome
