A 70 y/o woman falls on her elbow. She has marked bruising and tenderness of the upper arm. Neurovascular examination reveals wrist drop. What injury do you suspect?
Fracture of the shaft of the humerus.
Explanation: This is common in the elderly. The extensive bruising is characteristic. Diagnosis is supported by wrist drop which has been caused by radial nerve injury where it lies in the spiral groove of the humerus.
