10 steps to fitting a URA
1 - ensure appliance details match patient
2 - check appliance matches the design
3 - inspect appliance and run finger over to check for sharp edges
4 - check wire work integrity
5 - insert into mouth and look for areas of blanching or soft tissue trauma
6 - check posterior retention (adam’s clasps)
7 - check anterior retention
8 - activate appliance
9 - show patient how to insert and remove
10 - book review appt in 4-6 weeks
10 instructions to tell pt about URA
1 - will feel big and bulky initially , this is normal and will pass
2 - may cause initial excess saliva but this will pass within first 24 hours
3 - may impinge on speak , practice reading a book out loud to help this subside
4 - may experience discomfort/ ache - this shows appliance is working
5 - should be worn 24/7 including sleeping and eating
6 - remove after every meal and clean with a soft brush
7 - remove and safely store in a container during contact sports
8 - avoid hard and sticky foods that may damage appliance and be cautious with hot foods and drinks as roof of mouth is covered - may burn
9 - missing appointments or not wearing appliance will significantly lengthen treatment
10 - provide emergency contact details
steps to create an adam’s clasp
1 - straighten wire
2 - create 90 degrees bends measured to size of tooth
3 - make arrow heads, as small as possible
4 - position arrowheads at 45 degrees to bridge, using pliers across arrowheads
5 - using cast to orientate, push wire until perpendicular to bridge, will take 2 movements
6 - place plier tips as close to arrow head as possible and bend the wire till it goes between the teeth
7 - leave a straight portion going interprox, then mark where bend towards palate should begin
8 - bend towards palate
9 - create tag
10 - repeat on other side
