Oral Manifestations of Systemic Disease Flashcards
Classification of oral manifestations of systemic disease
- Soft tissues/Hard tissues
- System by system
- Ulcerative/non-ulcerative
Oral mucous membrane colours and what disease they suggest
Pallor: anaemia
Yellowish tint: Jaundince
Brown pigmentation in caucasians: Addison’s disease
Which diseases can cause Gingival or mucosal bleeding
- Purpura present
a. Acute leukaemia
b. AIDS
- May be subcutaneously and submucosally
- Excessive gingival bleeding may be a feature - No purpura present
a. Disorders of clotting
- Excessive gingival bleeding may be a feature
Which diseases can cause acute gingivitis
- Acute leukaemia
- Immunodeficiencies
a. uncontrolled diabetes
b. agranulocytosis (blood disorders with low wbc)
c. AIDS - Vitamin deficiencies
a. scurvy
b. pellagra (Vit B)
Which diseases can cause stomatitis
- Non-ulcerative
a. Sjogrens
b. Candidosis - Ulcerative
a. RAS

d. drug reaction
Which haematological diseases have oral associations?
- Haematinic deficiency (ferritin, folate, B12)
- Cyclical neutropenia
- Leukaemia
Which dermatological diseases have oral associations?
- LP
- MMP, bullus pemphigoid
- Linear IgA disease/ Dermatitis herpetiformis
- Pemphigus
- Erythema multiforme
Which GI diseases have oral associations?
- Coeliac (genetic condition involving hypersensitivity of SI to gliadin (in gluten) - results in atrophy and malabsorption)
oral presentation: ulceration, glossitis burning mouth, angular stomatitis - Ulcerative colitis
- Crohn’s disease
- Orofacial granulomatosis
- sarcoidosis
- Orofacial granulomatosis
- Melkersson-Rosenthal syndrome (labial and perioral swelling)
- Meischer’s Syndrome (labial swelling)
- Hypersensitivity to food-stuffs or preservatives
Commonest cause of cervical lymph node enlargement
- oral sepsis
- may also be first sign of disease of reticuloendothelial system: infective mononucleosis, AIDS, Hodgkin and non-hodgekin lymphoma
Which diseases can cause glossitis
- Anaemia
- most common identifiable cause
- often due to iron deficiency
- latent deficiencies are also important - Vitamin deficiencies
- Riboflavin deficiency and pellagra (vit b)
- Malabsorption syndrome
Which diseases can cause enlarged tongue
- acromegally
- amyloidosis (build-up of an abnormal protein called amyloid in organs)
- granulomatous condition
- neurofibromatosis (tumour along nerves)
- hamartoma (benign tumour)
- neoplasia
- haemangioma (vascular tumour derived from blood vessel cell types)
- Lymphangioma (benign malformations of the lymphatic system)
What can cause abnormal tooth colour
- Tetracycline: yellow-brown/grey
- Fluorosis: opaque white or brown patches
- severe jaundice: yellow-greenish
- porphyria: purplish red
- Dentinogenesis and osteogenesis imperfecta: purplish or brownish but with increases translucency
What can affect enamel formation
- Low birth weight
- Febrile illness (illness causing acute fever)
- Systemic disturbance
- Malabsorption/nutritional deficiencies
- Medication
What can cause enamel hypoplasia or hypomineralisation
- congenital syphilis
- severe childhood fevers
- severe fluorosis
- severe rickets
- hypoparathyroidism
all cause grooving pitting of enamel
Which diseases can cause abnormal tooth surface loss
intrinsic acid: GORD and bulimia
