Potential interactions with methadone
Benzodiazepine (respiratory depression).
Should not be used with suboxone or naltrexone.
Haloperidol, ziprasidone, pimozide (QT)
Carbamazapine (inducer)
Fluoxetine, fluvoxamine, tramadol
Ingredients of Suboxone
Buprenorphine and naloxone
Dosing of buprenorphine.
Initial 2-4 mg, max 8 mg on the first day. Titrate dose to prevent withdrawal symptoms.
Maximum dose of Suboxone
24 mg daily
Condition that must be present to start Suboxone
Patient must be in opioid withdrawal already otherwise withdrawal will be precipitated.
Dose titration of methadone
Initial dose of 30 mg. Titrated usually to 80-120 mg / day. Rare to exceed 120 mg.
Screening question for drug dependence
How many times in the past year have you used an illegal drug or used a prescription medication for non-medical reasons? (3 or more in the past year is predictive of drug dependence).
Outcomes of abstinence-based treatment
Poor; about 10% at one year.
Suboxone compared to methadone
Evidence is more favourable for methadone, but suboxone is easier to use.
Effects of methadone on the heart
Increases QT interval
These medications are relatively safe to use with methadone
Sertraline, venlafaxine, mirtazapine, bupropion, valproic acid.
