Name the three types of cesarean births
Scheduled
Emergent
Emergency/crash
What are the reasons as to why a woman would have a scheduled cesarean?
- HIV risk
- Multiple gestations
- Breech presentations
- Previous C-section
What is the emergent cesarean
Occurs when a woman labors and “failed to progress”
Often called at the end of a day with “no progress” and before the day team leaves
Are schedule cesareans emergency?
They are not emergency and therefore treated like any other surgical procedure
What is an emergency or Crash
Life threatening for baby not so much mother
ACOG guidelines state get “baby out in 30 minutes from the time “called”.
At most high risk centers can do it in 5-7 minutes
What is the nurses role during the preoperative phase
- ID the patient in the system
- Monitor FHT
- Prep for procedure: IV, baseline vitals on mom and baby.
- Notify neonatal provider and anesthesia
How many family members can be in the OR with mom?
Only 1 member
When does the intraoperative circulating nurse start their role?
Once in the OR and the first incision is made
Our role changes from patient support to technician to room controller
What is the nurses role during intraoperative phase?
- Monitor FHT
- Foley catheter
- Instrument count with scrub tech
- Document in detail all events
- Maintain count of instruments and lab sponges
- Document EBL
Can you administer antibiotics during the operation?
No you don’t administer the antibiotics you only make sure it is hung
What is the CRNA responsible in the OR?
They hang the blood
What types of details do you document?
The names of everyone
Time of 1st incision
Time of closing incision
Time in/out of OR
When the baby is born how often do you do vitals?
Q15
What is the nurses role during post op phase?
- Vital signs Q15 min
- Lochia amount
- Maintain IV patency
- Monitor I & O
- Assess pain level
