Health promotion is?
A strategy for improving the populations health by providing the information and tools so they can make informed decisions.
Rehabilitation is?
The process of restoring a person to the highest level of functioning through the use of therapy and exercise. It begins at the time of injury. Rehab may involve measures of self care, communication, mobility,elimination, positioning and prevention of decline.
Goals of rehab depend on the ct’s condition and circumstances, There are four ways…..
1) restore function to former level.
2) Improve functional ability
3) Learn new skill
4) prevent further disability
The rehab team treats all dimetions of health…what are they?
1) Physical- ADL’s, ROM, re-learn self care.
2) Emotional-provide counselling and assistance for re-entering the community.
3) Intellectual- thinking, speech, memory. measures to improve cognitive function
4) Spiritual- counselling from spiritual advisors.
Protheses are?
artificial replacment for missing body part. powered by the ct’s musc;es in the stump.
Orthoses is?
an apparatus used to alighn, prevent or correct deformities.can be a brace or splint.
Ways of supporting people in rehab are?
Provide privacy (close curtains &doors), provide encouragment, praise their improvments and strengths, be a good listener, provide emotional support.
**Restorative care is?…
care that helps a person regain health, strength, & independence
Prevention is?
care or support which attempts to prevent or stop deterioration & excess disability.
Learning is?
a change in behaviour which results from the aquisition of new knowledge or skill.
Motivation is?
Desiring something and putting energy towards it
Developmental task is?
A skill or learning process that is expected to occur at a particular age.
Development is?
The unfolding of characteristics and abilities in an orderly way.
What does the care plan consist of?
Dx, Tx, Vs, personal care, mobility, allergies, home making needs, saftey precautions, DOB, Rm #, Code status.
Medical Dx is made by?
A doctor. ie any health condition ie cancer, PCOS, diabetes, asthma, etc.
Nursing Dx is made by a nurse and does what?
Made by nurse and treats syptoms of disease or med problem diagnosed by a doctor.
What does a PSW contribute to making a care plan?
discuss and report changes in ct.
A care confrence is?
a meeting that identifies needs and prefrences of ct, deals with ct issues, establishes goals and provide consistancy of care.
who is involved in a care confrence?
Physician- discuss meds and Tx plans.
Ct- raise problems and concerns
family- assist with problem solving
nurse- mediate, organize plan of care, raise issues and problems.
PSW- relay problems & observations, contribuate suggestions and help problem solve.
What is a care plan?
A document that details the care and services a Ct should receive. includes nursing Dx, goals & interventions. provides consistancy & continuity of care. its a form of contract b/w the healthcare team & ct.
What is a care confrence?
a meeting attended by the health care team, ct and family members to discuss the Ct’s care. Held on regular basis and when difficult situations arise, family members can ask questions, express feelings, and make difficult decisions.
Multidisciplinary team includes?
includes healthcare professionals from a variety of backgrounds and specialties who work together to meet Ct needs.
time managment tips are?
1) set a priorities
2) discuss and agree upon routine with ct
3) gather all suplies before hand
4) use time well, start with tasks that have waiting periods ie. laundry
5) Set time limits for each task and try to stick to them the best you can
6) focus on task at hand, try not to let mind wander
7) put ct’s needs first! remember your first priority is to promote the comfort, well being and safety of your ct.
Things to remember when documenting are?
always use ink, always date time, write neatly, only use approved abbrevs, use correct spelling and grammar, if mistake is made use a single line do not erase or white out, sign all entries with name, and title, do not skip lines, record only what YOU observe and preformed, nvr chart until after procedure is complete, be accurate precise and factual, Do not record assumptions or opinions, record in logical manner, be descriptive, avoid terms with more than one meaning, report any and all changes, record all safety measures.
