Working at heights unprotected

Overload. Workplace factors must fit the worker. Workplace designs and controls are designed to fit the capacities and threshold limit of the worker. Accidents occur when limits and capabilities are exceeded.

Domino models are a chain of events that can result in loss. Intervention of events along the sequence could halt the process. Failure in the management system allows the domino stage to progress to incident stage.
What is overload?
Overload occurs when the full capacities of a worker are exceeded such as stress, fatigue, personal issues, pressure etc.
