What is obstructive shock
Blockage issue
Caused by a physical barrier such as cardiac tamponade, pulmonary embolism, or tension pneumothorax preventing ventricular filling or emptying.
Results in decreased cardiac output, poor tissue perfusion, and an increase in SVR (afterload)
Causes
Cardiac tamponade
Tension pneumothorax
Pulmonary embolism
Cardiac tamponade
Pericardial sac filled with fluid causes pressure to be exerted on the heart
Pericardial effusion can occur because of infection or from OHS (obesity hyperventilation syndrome)
Tension pneumothorax
Air within the pleural space puts pressure on lungs causing it to compress the mediastinum which can affect the heart
Pulmonary embolism
Clot in the pulmonary vasculature
Clinical symptoms
Related to decreased cardiac output
Hypotension
Tachycardia
Tachypnea
Decreased LOC
weak pulses
Pale, cool skin
Decreased urine output
Decreased bowel sounds
Nursing management
Treat the cause
Maximize oxygen delivery
Support hemodynamics

May help decrease workload and preserve heart tissue
-prepare for intubation and mechanical ventilation
Support hemodynamics
Vasoactive medication to increase BP
Vasopressors and Inotropes
