- fam hx
- lean body habitus
- lack of exercise/excessive exercise in the young
- glucocorticoids
- alcohol, smoking, low Ca2+, low VitD, high Na+, high phosphate (carbonated drinks), too much caffeine, high protein diet
- high homocysteine levels
