  • caucasion/asian background
  • early menopause
  • fam hx
  • lean body habitus
  • lack of exercise/excessive exercise in the young
  • glucocorticoids
  • alcohol, smoking, low Ca2+, low VitD, high Na+, high phosphate (carbonated drinks), too much caffeine, high protein diet
  • high homocysteine levels
