Gradations of Murmurs (p. 382)

A

Grade 1 Very faint, heard only after listener has “tune in”; May not be heard in all positions

Grade 2 Quiet, but heard immediately after placing the stethoscope on the chest

Grade 3 Moderately loud

Grade 4 Loud, with palpable thrill

Grade 5 Very loud, with thrill. May be heard when the stethoscope is partly off the chest

Grade 6 Very loud, with thrill. May be heard with stethoscope entirely off the chest

A

4+ Very brisk, hyperactive, with clonus (rhythmic oscillations between flexion and extension)

Scale for Grading Muscle Strength

A

Muscle strength is graded on a 0 to 5 scale:
0-No muscular contraction detected
1-A barely detectable flicker or trace of contraction
2-Active movement of the body part with gravity eliminated
3-Active movement against gravity
4-Active movement against gravity and some resistance
5-Active movement against full resistance without evident fatigue.
This is normal muscle strength
