Level 1
Minimum of hourly check
This is the minimum acceptable observation level for all inpatients and should include location of the patient, safety and well-being.
Level 2
Observations of the patient should be conducted at frequent intervals. The frequency of these is determined and agreed by MDT and dependent on level of risk exhibited by the patient.
Level 2:15 mins (yellow paper)
Level 2: 10 mins (blue paper)
Level 2: 5 mins (green paper)
Level 2: 10 mins intermittent- this is rarely used and involves a staff member checking the patient at any single point within a ten minute period. This becomes useful when the patient is harming themselves but aware that they are going to be checked at a specific time.
Level 3
Constant observation with close proximity
The patient must be in full sight of the staff member at all times, and the staff member needs to be close enough to the patient to enable effective intervention at any time if required.
Level 4
Constant observation within arm’s length
The patient must be in fill sight and within arm’s length of the nurse at all times, and the nurse needs to be close enough to the patient to angel effective intervention at any time required.
This is not always advised for monitoring aggressive patients as it leaves the observer at risk. It is normally initiated to reduce the risk of harm to self.
