What are RF for PP blues?
hx of depression, depressive symptoms in pregnancy, family history of depression, PMS, stress around child care, psychosocial impairment
How common is PP depression?
5-9% point prevalence; 10-25% lifetime risk
What is the timeline for PP depression?
Begins within 1 month of delivery but may diagnose up to 12 months
What is the definition of PP depression?
> = 5, nearly every day, most of the day for >=2 weeks:
- Depressed mood
- Loss of interest/pleasure
- Insomnia/hypersomnia
- Change in appetite or weight
- Psychomotor retardation/agitation
- Low energy
- Poor concentration
- Worthelssness or guilt
- Thoughts of death or suicide
RF for PP depression
Hx of depression, marital conflict, stressful life events in previous 12 months, lack of perceived social support, living w/o a partner, unplanned pregnancy, previous miscarriage, not breastfeeding, sick leave during pregnancy, congenital malformations, GDM, hx of bipolar d/o, family psych history
What SSRI treats PP depression?
Same as in gen pop except breastfeeding, in which case use sertraline or paroxetine
