Why do we use tocolytics
A
Reduce the strength and frequency of contractions in preterm labor (22-34 weeks)

Goals of tocolytics
A
- Delay delivery 48 hrs for steroids
- Provide safe transport of mom
- Prolong pregnancy as long as safe

NSAID/Cyclooxygenase inhibitor
A
Indomethacin
- 24-32 weeks prior to fetal surgery

CCB
A
Nifedipine
- first line 32-34 weeks gestation
- can see transient hypotension, flushing

Beta-2 agonist
A
Terbutaline
- 2nd line
- tachycardia, hypotension, dysrhythmias, hyperglycemia

Antenatal steroids?
A
Used for preterm labor to accelerate fetal lung maturity (<34 weeks gestation)
- betamethasone 12 mg IM q24hrs

Benefits of antenatal steroids
A
Lung maturity –> decreases neonatal respiratory distress syndrome, IVH, death

Magnesium for fetal neuroprotection
A
Not indicated if >32 weeks gestation

Magnesium for mom in pre-eclampsia
A
short term use for moms with pre-eclampsia for seizure prevention
