What are the components of the BPP?

A

Nonstress test (NST) combined with 4 observations made with ultrasonography:

  • fetal breathing
  • movement
  • tone
  • amniotic fluid volume

BPP scoring?

A

For each variable assessed with the BPP, a score of 0 (absent or abnormal) or 2 (present or normal) is given for a total of 10 possible points.

BPP score of 6 or less in full term baby. Next step in management?

A

A score of 6 is considered equivocal, as fetal acidemia and hypoxemia cannot be excluded. At term gestation, a BPP of 6 or less supports prompt delivery.

BPP score of 6 or less in preterm baby. Next step in management?

A

If preterm, a BPP of 6 or less supports repeat testing in 6 to 24 hours (as in the vignette), to minimize iatrogenic premature delivery of a healthy fetus.
