CNIII; Extraocular eye movement, raise eyelids, pupil constriction PERRLA, and 6 fields of gaze.
Dysfunction: Diplopia, Ptosis of the eye lid
Trochlear
CNIV; Donward/Inward Eye movement- PERRLA and 6 Fields of Gaze
Dysfunction; Convergent Strabismus and Diplopia
Trigeminal
CNV; Motor: Chewin- clench jaw. Sensory: Taste- Anterior 2/3rds of tongue- identify sweet/salty taste
Dysfunction: Decreased ability to chew; loss of facial sensation
Abducens
CNVI; Lateral Eye Movement- 6 Fields of Gaze
Dysfunction: Diplopia, Strabismus
Facial
CNVII; Motor: Facial Expressions- smile, puff out cheeks
Sensory: Taste- Anterior 2/3rds of tongue- identify sweet/salty taste
Dysfunction: Bell’s Palsy and Decreased ability to distinguish taste
Acoustic
CVIII
Hearing- whisper test
Equilibrium- close eyes and check balance
Dysfunction: Tinnitus- ringing in ears and Vertigo
Glossopharyngeal
CNIX
Motor: Swallowing-gag reflex with tongue depressor
Taste- Posterior 2/3rds of the tongue identify sweet/salty
Dysfunction: Loss of gag reflex and loss of taste
Vagus
CNX
Motor: Swallowing- movement of the palate & uvula when saying ‘ah’
Sensory: Phonation- guttural speech saying
Dysfunction: Difficulty swallowing and loss of voice or impaired speaking
Spinal Accessory
CNXI
Head movement- turn head left and right
Shoulder movement- shrug shoulders against resistance
Dysfunction: Inability to turn head left to right and difficulty shrugging shoulders
Hypoglossal
CNXII
Tongue Movement- move tongue up and down, side to side
Dysfunction: Dysphasia, tongue deviates to side
