Verify the Dr’s order of blood transfusion
To ensures the right patient receives the right intervention
Verify the completion of informed consent for blood transfusion
To ensures the right patient and help prevent errors
Verify Dr’s order and prepare pre - transfusion medication
To decrease the risk of allergic
Give brochures of blood transfusion
For patient cooperation
Check the EMR - lab result
For collection of blood
Explain the procedure of the blood transfusion and its purpose to the patient
To relieves anxiety and facilitate cooperation
Take of vital sign - temperature, pulse, respiration and blood pressure
As a baseline reading
Check of the following
* Completeness of Blood Collection Form (BK MIS 1152)
* Cannula e.g. site, size 18 gauge or 20 gauge
To avoid delay transfusion process
Taking the form and Blood Transport Boxes for collection and to collect the blood together with compatibility report form
To ensure the correct blood
Start infusion of 0.9% Normal Saline
To keep vein open
Check the following information on the pack is the same as the blood in the form Compatibility Report Form and Form Identification of the patient by 2 person (medical officer doctor’s and a nurse or 2 medical officer)
• Patient’s name
• Registration number
• Ward
• Blood type rhesus factor
• Number of blood packs
• No foreign bodies
• Expiry date
• The amount of blood
To prevent errors
Record the following information on the pack label
• The name of doctor’s medical officer
• The name of the nurse
• Date and time of starting transfusion
