NUR 146 CL - IV B Flashcards
IV B: Primary infusion should already be hung. Then, setup secondary
- Get medication and secondary infusion bag
- Verify compatibility between medication and diluent
- Use blunt needle to retrieve medication (Insert, inject air, invert, retrieve med)
- Inject med into bag, then label (Initials, date, time and med)
- Hang secondary bag
- Close roller clamp on tubing
- Invert and spike secondary bag
- Hang secondary higher than primary (attach hanger to lose PRIMARY)
- Use alcohol to clean Y-Port closest to primary bag, then connect secondary tubing
IV B: After setting up and attaching the secondary bag, prime the secondary tubing
Back prime
- Tubing should be attached to IV port; primary roller clamp closed
- Loosen secondary roller and lower the secondary bag
- Allow primary bag to back-infuse into secondary tubing
- CONTROL BACK PRIME WITH ROLLER CLAMP ON SECONDARY TUBING
- ONLY ALLOW 1/3 - 1/2 OF CHAMBER TO FILL, THEN SHUT CLAMP
- Raise secondary bag up
- ## Primary roller clamp should still be closed
IV B: After attaching and priming everything, now run the infusion
- All IV tubing (already, prior to priming)
- Open roller clamp
Gravity:
Get drip factor, divide by 4, count for 15sec
(Tubing is attached to IV port, so no medication is wasted
IV Pump BOTH:
- Program each solution as prescribed, if pump is capable
- Once you program the first, there will be an option at the bottom to add a second rate
IV Pump SINGLE:
- Program for secondary rate (medication)
- Then, primary will start when piggyback is completed
- Then, readjust infusion rate for primary solution
IV B: If you’re required to add medication to a pre-existing bag
Stop infusion
- If pump: Pause
- If gravity: Clamp tubing
Things to check:
- Verify correct amount of diluent
- Verify compatibility of med w/ diluent
- Verify enough time on bag to complete med
- Clean med port on IV bag
- Use blunt needle to inject medication into bag (Needle goes to sharps container)
- Agitate medicine in bag; then medication for bag
