Visual Acuity
Not correctable to 20/25 in at least one eye is disqualifying
Defective color vision is disqualifying. Waiver requests must include statement from members supervisor. Examiner must include evidence that primary and secondary colors can be discerned.
Requirements of Nuclear field duty Exams
Psychological and Cognitive
Any current or history of Axis 1 diagnosis is disqualifying
P-117 chap 15 103, discusses specific conditions, exceptions and waivers
Waivers
Disqualification and Waiver are essentially the same personnel action.
Requests sent from member’s CO via applicable ISIC or Type Commander and BUMED M-95 to the appropriate Bureau of Naval Personnel Code. Enlisted PERS 403 Officers PERS21
WEBWAVE2 is used to trasmit.
Periodicity of NFD exam
Every 5 years until 50 years old
Every two years age 50-60
Every year at age 60 or greater
Who can perform NFD exams
Physician, PA, NP with clinical privileges
Must be co-signed by UMO, RAM.
