Contour of Abdomen
Scaphoid, Flat, Rounded, Protuberant
Liver Span technique
Up to down is resonant to dull
Down to up is tympany to dull
LIVER
RUQ 5th intercostal
SPLEEN
LUQ 9-11th intercostal
Costovertebral Angle Tenderness
Indirect fist percussion on the back side of the patient to percuss kidneys. 12th rib. Tissues vibrate instead of making a sound.
Shifting Dullness
Test for ascites. Percuss down the side of the abdomen and if fluid is present then the note will change from tympany to dull when you reach it.
Rebound Tenderness
AKA Blumberg Sign. Push hand down at 90 degrees slowly and deep. When you let go there will be sudden rebound. Normal or negative result is no pain.
Inspiratory Arrest
AKA Murphy Sign. Specific to the gallbladder. Put fingers under the liver border and have patient take a deep breath. Patient should not have pain.
Iliopsas Muscle Test
To test for appendicitis. Lay supine, lift right leg straight up. RN pushes down on right thigh. If there is pain, then there is appendicitis.
Romberg Test
Person stands with feet together and arm at the sides, with eyes closed for 20 seconds. Person should not lose balance.
Spinothalamic Tract
pain, temperature, and light touch
Posterior Column Tract
Vibration, Position, Tactile discrimination (fine touch)
Tacticle Discrimination Tests
Stereogenesis, Grapesthesia, two-point discrimination, extinction, point location
Extinction
Close eyes, touch opposite sides of the body and patient will tell you where you are touching
Point Location
simulataneous touch patient’s body and they tell you where
Reflex Grading Scale
0 = absent 1 = diminished 2 = normal 3 = brisker than average 4 = very brisk, hyperactive
Clonus
Short, jerky contractions, not controllable
Biceps Reflex
