The Hackett grading system
Grade 0: Spleen is not palpable, even on deep inspiration.
Grade 1: Spleen is just palpable below the costal margin, usually only on deep inspiration.
Grade 2: Spleen is palpable, but not below a horizontal line halfway between the costal margin and the umbilicus.
Grade 3: Spleen is palpable more than halfway to the umbilicus.
Grade 4: Spleen is palpable below the umbilicus.
Grade 5: Massive splenomegaly, extending significantly below the umbilicus.
