Candida
Bacterial Vaginosis
What is vulvovaginal candidiasis caused by (VCC)
Candida albicans 90% of the time
Symptoms of VCC
Vulval pruritis and burning
Non-offensive smelling discharge
External dysuria and superficial dyspareunia
Erythematous rash
Cyclical sometimes
Signs of VCC
Erythema
Fissuring
Curdy white discharge
Oedema of vulva
Satellite lesions
Non offensive smell
Diagnosis of VCC
Based on clinical signs and symptoms, testing for other causes before starting empiricle treatment.
Routine microscopy to identify candida spores, pseudohyphae and neutrophils
What classes as recurrent VCC
At least 4 episodes per 12 months with 2 episodes confirmed on microscopy or culture when symptomatic
Treatment of asymptomatic patients in VCC
No treatment
Treatment of symptomatic patients in VCC
Oral azoles - fluconazole capsule 150mg single dose by mouth. Econazole, itraconazole, miconazole, fenticonazole
Topical regimen for symptomatic patients for VCC
Clotrimazole pessary 500mg dose PV. Or other vaginal creams
Two less common causes of VCC
Candida glabrata and candida krusei
Where are swabs taken from in candida or BV
High vaginal swab, from discharge or vaginal wall
Symptoms of BV
Fishy smelling discharge, no associated itching, irritation or swelling
Signs of BV
White/grey discharge coating vaginal walls with or without odour
No associated inflammation
