COPD exacerbation, cariogenic pulm edema, obesity

Hypoxic RF, post-extubation hypoxia, acute pulm edema, DNI
When is NIPPV indicated
COPD exacerbation, CHF exacerbation, post-op
What is the main goal of NIPPV
Reduction in Inspiratory muscle work and avoidance of respiratory muscle fatigue
Indications for CPAP on NIPPV
Hypoxic not hypercapnic patients, improves alveolar edema and increases functional residual capacity
What is the PAV (positive airway ventilation) (Trilogy) on NIPPV
Delivers Inspiratory pressure proportional to the patients effort. Based on patient generated volume and flow. No present volume or pressure
What is AVAPS on NIPPV
Average Volume Assured Pressure Support. Can set a tidal volume along with pressures.
Who are good vs bad candidates for NIPPV
GOOD: Able to protect airway, control secretions, awake and alert, cooperative
BAD: unstable hemodynamics, uncooperative, unable to protect airway, facial trauma, GI bleed, aspiration
What are the disadvantages of NIPPV
Slow gas exchange, gastric distention, air leak, skin/eye irritation, aspiration
