It’s a spectrum of diseases From least severe to most severe Steatosis Steatohepatitis Fibrosis Cirrhosis
Etiopathogenesis of non alcoholic fatty liver disease
Due to fat deposition in the liver Unrelated to alcohol or viruses It affects individuals with metabolic syndrome that is ; Obesity Hypertension Diabetes Hypertriglyceridemia Hyperlipidemia
Insulin resistance results in insulin receptors to be less responsive , in result the liver goes into increase fat storage and decrease fatty acid production that means decrease of lipids release into the blood stream in the form of lipoproteins
And increase of fatty acids uptake in blood called steartosis causing fat droplets to form around hepatocytes, causing nucleus to appear at the edge of cell
Overtime the liver is vulnerable to degradation
Lipid membrane damaged causing cell death causing stearrthoe hepatitis
Steatohepatitis can cause stellate cells to lay down fibrotic tissue that is fibrosis
Overall architecture of the liver changes causing cirrhosis
Clinical picture of non alcoholic fatty liver disease
Even at advanced stages the patient may be asymptomatic
Symptoms include ; Fatigue Malaise Pain Jaundice
Diagnosis of non alcoholic fatty liver disease
Hepatomegaly on physical exam
Ascites
Increase ASAT, ALAT in biochem
US abdominal
Ct MRI
Liver biopsy - a liver with more than 5% fat content is considered abnormal
Treatment of non alcoholic fatty liver disease
Steartosis and steatohepatitis are reversible with a healthy diet, active lifestyle,meds that control blood glucose
