Placental Abruption

A

Definition: Premature separation of normally implanted placenta
Risk Factors: Maternal: AMA, cocaine, smoking, alcohol, abnormal elev AFP

OB risk: chorio, FGR, hx abruption, short interconception, HTN, Multi gestation, PROM, Poly, Fibroids, Thrombophioia, hypothyroid, asthma, hx c/s
Acute: trauma, ECV
DX: US rarely helpful ,Dx based on clinial s/s
Management: Expedite birth near term
At risk for coagulopaty and hypovolemia
May cause Couvelaire uterus

A

Def: Placenta implants over cervical os, dx routine US
RF: Previous uterine scaring (C/S, uterine surgery)
Maternal age
Multigestation Smoking ,Cocaine ,Short interconception
Terminations ,Reproductive tech
S/S: painless bleeding
DX: Trans vag US
Placenta close to not covering os (2 cm) term low lying
Management: Vag birth contraindicated ,No sex with vaginal penetration or orgasm

DX: US, risk based US for previa

S/S: PPH risk inc 20-75 min
PPH:
Dx: 3rd stage 20 min < AMTSL, 60 min < EMTSL
Mng: consult/collab at 20 min/60 min, upright position, nipple stim, empty bladder, manual remove @ 30 min
