How do you elicit Nikolskys sign?

A
  • Apply lateral pressure with thumbs or fingertips on peri lesional skin
  • Results in shearing force that dislodges upper layers of epidermis from lower epidermis
  • Positive if upper epidermis separates from lower
  • Helps distinguish pemphigus and bullous pemphigoid

  • Dislodgement of intact superficial epidermis by shearing force
  • Indicating plane of cleavage in skin at dermal -epidermal junction which induces a bulla
