How do you document a NEWS 2 chart?
Dots in relevant boxes.
Only use figures when required.
Legibly write the name and DOB of the patient, and the date and time.
What are A + B on a NEWS 2 chart?
Resp rate.
SpO2 (including chronic hypoxia patients).
If they are on oxygen.
What is C on a NEWS 2 chart?
Blood pressure.
Pulse.
What are D + E on a NEWS 2 chart?
Changes in neuroresponse.
Temperature.
What else is documented on a NEWS 2 chart?
Blood glucose.
Pain score.
Fluid balance.
Escalation of care.
How do you respond to NEWS 2 scores?
0 - ward-based response.
3 in anything - urgent ward-based response.
5-6 - key threshold for urgent response.
7+ - urgent or emergency response.
How is a sepsis screening tool used?
Patient with a NEWS score >5 or of clinical concern or suspected neutropaenia.
What is a CNS observation chart?
Used in acute care areas.
Considers coma, limb movement and seizures.
What is a SPICT?
A guide to identifying people at risk of deteriorating health and dying.
What is an escalation plan?
- Inform the nurse in charge / FY1.
- Inform the advanced nurse practitioner / FY2.
- Inform a doctor.
- Inform a consultant or senior doctor.
