Describe the pathway of getting from heme to bilirubin
A
heme –> (heme oxygenase) –> biliverdin –> (biliverdin reductase) –> bilirubin

A
Bilirubin is conjugated in hepatocyte by UDPGT & is excreted into bile

what happens to bilirubin in an infant gut when excreted into bile?
A
hydrolysed back into unconjucated bilirubin and re-enters enterohepatic circulation

How is bilirubin eliminated?
A
- cecal bacteria converts it to stercobilin

A
- immaturity of liver UDGPT
- Lack of cecal bacteria so more re-enters enterohepatic circulation
- relative increased load of RBCs
