What are the components of each of the parts of the APGAR score
Points from 0 | 1 | 2
- Appearance: cyanotic/pale all over | peripheral cyanosis only | pink
- Pulse: 0 | <100 | 100-140
- Grimace: no response to stimulation | grimace or weak cry when stimulated |cry when stimulated
- Activity: floppy | some flexion | well flexed and resisting extension
- Respiration: absent | slow and irregular | vigorous cry
When is the newborn assessment done?
In first 24 hours and repeated at 6-8 weeks
What is the newborn assessment for?
- to identify abnormalities
- to provide health education + parental reassurance
Before starting newborn examination what should be done?
- examine maternal + baby’s notes incl. birth weight, height, any known issues + APGAR
- ask about maternal and antenatal history - USS + bloods
- social history - child protection?
- ask parents if they have any concerns or problems feeding
- ask if baby has passed urine or meconium yet
What are the three component of a successful Moro’s reflex?
Abduction of arms
ADduction of arms
Startled look/crying
Outline Moro’s reflex
- one hand on baby’s head to slightly lift from table
- other hand holds baby’s arms across chest
- release to allow baby to fall briefly
- abduction, adduction and crying should occur
What does an absent/abnormal Moro’s reflex indicate?
- unilateral: shoulder or brachial plexus injury
- bilateral: brain or spinal cord damage
What is suggested if the Moro’s reflex persists past 4/5 months?
Severe neurological problem
Explain the Barlow test
- hold baby’s thigh near hip
- with gentle posterior/lateral pressure attempt to dislocate femoral head
- there should be no rotation
- if the hip dislocates, a clunk can be felt > then do Ortoleni test
Explain Ortoleni test
- complete after Barlow test
- hip is abducted
- pressure is applied to proximal thigh from behind
- attempt to relocate femoral head
- clunk felt if successful
