Hereditary hemorrhagic telangiectasia
syndrome resulting in AVMs that include nose in >95%, skin in >80%, GIT in 30%, lungs, liver 30% and brain. Elevated CRP in setting of intracranial ring enhancing lesion sensitive indicator of brain abscess.
Management of ruptured intracranial aneurysm?
Endovascular coil embolization or open craniotomy/clip placement reduces the risk of re-bleeding.
Treatment for symptomatic Chiari malformation and syringomyelia
Suboccipital craniectomy
Next step if patient p/w right-sided hemiparesis and dilated nonreactive pupil on the left.
Suspect compression from the left side - transtentorial herniation as brain bulges out of cranium through the tentorial notch
FUNDOSCOPIC EXAMINATION for the presence of PAPILLEDEMA
