Neurology Floors Flashcards
What is the BO goal in acute ischemic stroke?
Treat if:
- BP >220/120
- MI
- Malignant HTN
- > 185/110 if IV thrombolysis expected
Blood glucose goal for acute ischemic stroke?
60-180
cerebral edema peak after an acute infarct
2-3 days but can occur upto 10 days
Decrease ICP
- Elevate HOB
- IV mannitol
- Water restriction
Predictor of detoriation requiring hemicraniotomy
DWI brain volume of infarction
Stroke mimicking labyrinthitis
Cerebellar stroke
Decompression surgery for cerebellar stroke
Sub occiital craniotomy
TPA dose for acute ischemic infarct
0.9 mg/kg —> 10% bolus followed by rest over 1 hour
Does rTPA improve mortality?
No
Benefit of rTPA administration
Reduction in deficits
TPA window
3 to 4.5 hours
RTPA window in the US
0-3 hours
Study to select patients for thrombolysis beyond the TPA window
DWI-FLAIR and CT/MR perfusion study
Window for mechanical thrombectomy
24 hours
Antiplatelet for acute stroke
Aspirin
