Neuroleptic Malignant Syndrome Flashcards
1
Q
What is a neuroleptic malignant syndrome (NMS)?
A
Life threatening reaction to neuroleptic or antipsychotic
2
Q
What is a neuroleptic?
A
Another name for antipsychotic
3
Q
What are some common symptoms of NMS?
A
- High fever
- Rigid muscles
- Variable BP
- Sweating
- Tachycardia
4
Q
What are complication of NMS?
A
- Rhabdomyolysis
- Hyperkalaemia

- Seizures
5
Q
What are the main causes of NMS?
A
- Typical antipsychotics
- Atypical antipsychotics
- Phenothiazines - chlopromazine
- Butyrophenones - haloperidol
- Rapid decrease of L-dopa
- dehydration while on neuroleptic
6
Q
What is the pathophysiology of NMS?
A
- Blockage of D2 receptor
2, induce rapid glutamate release - causes catatonia, neurotoxicity and myotoxicity
- Blockage of D2 receptors in basal ganglia can cause parkinsonism
7
Q
What is the management of NMS?
A
- stop offending medication
- Agressive cooling blankets and ice packs
- IV fluids to prevent renal failure from myoglobin
- Use dantrolene, bromocriptine and diazipam
8
Q
What are risk factors for NMS?
A
- Rapid dose
- Long acting
- Dehydration
- Renal Failure
9
Q
What laboratory investigation can help differentiate between
A
- Increased WBC
2. CK from rhabdomyolysis

A
- Heat stroke
- Malignant Hyperthermia
- Seretonin Syndrome
- Drugs - cocaine, amphetamines
- encephalitis
