What is the pathophysiology of NMS?

A

Systemic dopamine deficit usually due to antipsychotic dopamine blocking drugs eg haloperidol, chlorpromazine, prochlorperazine, metoclopramide

What are the symptoms of NMS?

A

1) altered GCS
2) autonomic instability
3) Muscle rigidity (parkinsonism)
4) high temp

How can NMS be differentiated from serotonin syndrome?

A

-greater degree of muscle rigidity and less common to have hyperreflexia
-slower onset
-can progress to severe encephalopathy, rhabdo & MOF

How do you manage NMS?

A

Supportive measures
I&V - if low GCS
CVS support
Fluids
Coolling
Stop causative drug
May need EEG to r/o NCSE
