Lateral Medullary Syndrome (Wallenberg Syndrome)

A

Loss of pain and temperature over contralat. body
Loss of pain and temp over ipsilat. face
Hoarseness, difficulty swallowing, loss of gag reflex (nucleus ambiguus damage)
Vestibular nuclei damage–vertigo, nystagmus, n/v
Inferior cerebellar peduncle damage–ipsilat. cerebellar defects

A

Ctontralat. spastic hemiparesis
contralat. tactile and kinesthetic defects
tongue deviates to side of lesion
damages to Corticospinal tract, medial lemniscus, and hypoglossal nerve

A

Ipsilat facial nerve paralysis
ipsilat. limb and gait ataxia
ipsilat. loss of pain and temperature sensation from the face
contralat loss of pain and temperature sensation
ipsilat horner syndrome
Damage to: facial nucleus and nerve fibers, middle cerebellar peduncle, spinal trigeminal nucleus and nerve fibers, spinothalamic tract, descending sympathetics

A

contralat. spastic hemiparesis
contralat loss of light touch/vibration/proprioception
ipsilat. internuclear ophthalmoplegia
gaze away from lesion
ipsilat. paralysis of lateral rectus mm
Damage to: coticospinal tract, medial lemniscus, MLF, pontine gaze center and abducens nucleus

Anterior midbrain infarction from occlusion of paramedian branches of posterior cerebral artery
cerebral peduncle lesion–>dysphagia, dysphonia, dysarthria, contralateral spastic hemiparesis
possible oculomotor nerve pasty
