What are you going to do if you get the neuro lab?
1) Sanitize hands
2) evaluate orientation (person, place, time, situation)
3) perform cranial nerve exam 2-12
4) sensation upper and lower bilateral (pain and/or vibratory sensation
5) DTR (one upper, one lower)
6) perform cerebellar exam (fiber to nose, heel to shin, or rapid alternating movements)
7) Evaluate plantar response
8) special test
How do you evaluate person, place, time, or situation?

How do you evaluate CN 2-12?
DO NOT DO 8,
DONT DO taste for 7 and 12
CN2 + 3 –> direct and consensual pupil constriction (Check both eyes)
3, 4, 6 would be cardinal signs of gaze –> H test
Nystagmus and accomodation
Trigeminal MOTOR–> contact their masseter muscle and have them flex their jaw, move jaw side to side
Trigeminal SENSORY –> assess v1, v2, v3 and ask if its dull or sharp with the q tip
7 –> raise eyebrows, smile real big, frown, close eyes really tight and don’t let them open it.. puff up cheeks
8 –> whisper test –> say numbers, letters.
9 + 10 –> gag reflex.. or say AHH and have them see if their soft palate rises up.. also have them swallow and they’re talking to you.
11 –> shrug shoulders and have them move face into your hand for SCM
12 –> stick out tongue, Side to side, up and down, tongue strength inside teeth
What are you going to do for DTRs?
Biceps –> c5
Achilles –> S1
how are you testing for cerebellar function?
put palms on thighs and have them rapidly alternate
or finger to nose as you move your finger around, switch fingers
heel to shin and run it down and up, bilaterally
what specialty test are you going to do?
Romberg –> feet close together, one hand in front one hand behind –> close eyes and a little bit of swaying is normal
pronator drift –> have them hold their hands up, seeing if they drift away or pronate.
Sensory dermatomes UE and LE
use pointy part of cotton swab and do different points. ask them where they felt it.
C8 = deltoid C6 = thumb
Vibration sensations?
tuning fork vibrating and on joint
identify if they feel the vibrations on their locations
wrists, arms, ankles, toes
Proprioception (I don’t know if this is needed)
have their hand rest on their thigh and hold their finger on the sides of the finger –> pull it up then down or whatever with their eyes closed and have them say where it is
do it bilaterally, hands and feet
Plantar response?
babinski –> bottom of tuning fork at the calcaneus and go towards the big toe
normal is toes don’t move or flex forward
abnormal is if they fan out and extend upwards.
