What is the action of Aspirin?
It is a non-selective cyclooxygenase inhibitor that decreases the formation of prostaglandins involved in the production of inflammation, pain, and fever. INhibits platelet aggregation.
What are the main uses of aspirin?
Pain, inflammation, prophylactic for Ischemic strokes and MI’s
What are the four main contraindications of aspirin?
- Hypersensitivity to salicylates
- Pregnancy
- Hx of Gi ulcers, peptic ulcers, or any bleeding disorders
- Pediatric with recent viral infection (Reye syndrome)
What are the main side effects of Aspirin?
- Increased bleeding poteintial
- Epigastric distress
- heartburn
- nausea
What are the s/s of aspirin toxicity?
- Resp. alkalosis the progresses into resp depression and acidosis
- Hyperthermia
- Sweating
- dehydration with electrolyte imbalance
- tinnitus
- Headache
What are the 6 main nursing implications for aspirin?
MSTASP
- Give with milk or water to decrease gi irritation
- med safety to parents with children
- High tox potential in OA’s and Peds
- Avoid use with alcohol to decrease gi irritation
- stop use 1 week before surgery
- Determine the purpose
What is the action of Heparin?
Enhanced the inhibitory actions of antithrombin essentially blocking the conversion of prothrombin to thrombin and fibrinogen to fibrin
What are the 5 main uses for Heparin?
- Rapidly treats and prevents DVT, PE’s, and Emboli in Afib
- Disseminated intravascular coagulation (DIC)
- The preferred anticoagulant in pregnancy (does not cross the placenta and does not enter breast milk)
- Pts on Heart-lung machines and Dialyzers
- used as an adjunct to thrombolytic therapy of acute MI’s
What are the three main precautions/Contraindications of Heparin?
- Bleeding tendencies (Hemophilia, Dissecting aneurysm, peptic ulcer)
- Thrombocytopenia, uncontrollable bleeding, threatened abortion
- Postoperative patients (esp. eye, brain, spine)
What are the three main side effects of heparin?
- Injection site reaction
- Heparin-induced thrombocytopenia
- Spontaneous bleeding
