Neuro Diagnostics Flashcards
How do you diagnose GBS? What will you see
LP
CSF: high protein with normal WBC
How do you dx Myasthenia Gravis
- +Acetylcholine receptor antibodies
- Tensilon (Edrophonium) test: Rapid response to IV Edrophonium
- Ice pack test on eyes (improves ptosis)
Test of choice for confirming MS?
MRI with gaolinium :
White matter plaques (hyperdensities) with high intensity is hallmark finding
What is seen on LP in a patient with MS
Increased IgG (Oligoclonoal bands) in CSF
Classic CSF finding of what?
High IgG (oligoclonal bands)
MS
Classic CSF finding of what?
High protein with normal WBC
Guillain Barre Syndrome
Classic CSF finding of what?
- High protein
- increased WBC (primarily polymorphonuclear neutrophils)
- Decreased glucose
- Gram stain positive
Bacterial Meningitis
Classic CSF finding of what?
- Normal glucose
- increased WBCs (lymphocytes)
- <100 (normal or mildly increased) protein
- Negative gram stain
Viral (aseptic) meningitis
Classic CSF finding of what?
- Decreased glucose
- Increased WBCs (lymphocytes)
Fungal or TB meningitis
Classic CSF finding of what?
Increased CSF opening pressure, otherwise normal
Idiopathic intracranial HTN (pseudotumor cerebri)
Classic CSF finding of what?
Xanthocromia, blood in CSF
Subarachnoid hemorrhage
Dx of concussion-
Study of choice for evaluating acute head injuries?
CT
Dx concussion-
Study of choice if prolonged sxs >7-14d, or with worsening sxs not explained by concussion syndrome
MRI
Dx Alzheimers
CT- cerebral cortex atrophy on CT scan
